New Patient
Patients will need to submit their ARIZONA MMJ Patient card and a secondary form of ID (i.e. Arizona State ID/Drivers License, or Passport) via text or email. KEEP READILY AVAILABLE; both forms of IDs will be verified upon each delivery.
Text or Email Hermes Delivery Service with all the required information along with your order and our dispatcher will be in contact via text or phone call shortly after to verify delivery.
Patients will also be required to fill out a "New Patient" form, if not an already existing patient. Our delivery team will have copies available to fill out upon delivery if needed.
OR patients may come into one of our two Arizona licensed dispensary locations, and upload patient information in person. (Same documentation will be needed).
